What’s the difference between Medicare Part A and Part B when it comes to costs?
Medicare Part A and Part B have different premiums, deductibles, and coinsurance amounts.
Part APart B Premiums. Most people get premium-free Part A. You generally qualify for premium-free Part A if you’re 65 and already get retirement benefits from Social Security or the Railroad Retirement Board. You can also get premium-free Part A if you’re under 65 and got Social Security or Railroad Retirement Board disability benefits for 24 months. Most people pay a premium for Medicare Part B. The standard Part B premium is $170.10 in 2022, or higher depending on your income. Deductibles In 2022, the Medicare Part A deductible is $1,556 for each benefit period. The Medicare Part B deductible is $233 a year in 2022. Coinsurance You pay a Part A coinsurance (a set dollar amount) for certain days in the hospital beyond day 60. This amount is $389 per day for days 61-90, and $778 per day for days 91 and beyond, in 2022. However, if you use up all 60 of your “lifetime reserve days,” you could have to pay all costs. You pay 20% of the Medicare-approved amount for most doctor services under Medicare Part B. You generally won’t know the actual dollar amount until you receive a bill.
What is the difference between Medicare Part A and Part B coverage?
Medicare Part A and Part B cover different things, and there is generally no overlap in coverage. Here are some examples – this is not a complete list.
Part A coverage Part B coverage Your hospital room and hospital meals

Prescription drugs received in the hospital
Skilled nursing facility care
Hospice care
Limited home health care Doctor visits both to primary care doctor and specialists
Some vaccines, including the flu shot
Mental health services
Annual physical exam

Emergency ambulance transportation
Physical, occupational, and speech therapy
Some preventive exams, tests, and screenings
Medicare Advantage Plans cover all Medicare services
Medicare Advantage Plans provide all of your Part A and Part B benefits, excluding clinical trials, hospice services, and, for a temporary time, some new benefits that come from legislation or national coverage determinations. Plans must cover all emergency and urgent care and almost all medically necessary services Original Medicare covers. If you’re in a Medicare Advantage Plan, Original Medicare will still help cover the cost for hospice care, some new Medicare benefits, and some costs for clinical research studies.
The plan can choose not to cover the costs of services that aren't
medically necessary under Medicare. If you're not sure whether a service is covered, check with your provider before you get the service.
With a Medicare Advantage Plan, you may have coverage for things Original Medicare doesn't cover, like fitness programs (gym memberships or discounts) and some vision, hearing, and dental services (like routine checkups or cleanings). Plans can also cover even more benefits. For example, some plans may offer coverage for services like transportation to doctor visits, over-the-counter drugs that Part D doesn’t cover, and services that promote your health and wellness. Check with the plan before you enroll to see what benefits it offers, if you might qualify, and if there are any limitations.
Plans can also tailor their benefit packages to offer additional benefits to certain chronically ill enrollees. These packages will provide benefits customized to treat specific conditions. Although you can check with a Medicare Advantage plan before you join to see if they offer these benefit packages, you’ll need to wait until you join the plan to see if you qualify.
Most plans include
Medicare Drug Coverage (Part D). In addition to your Part B premium, you usually pay a monthly premium for the Medicare Advantage Plan.
In 2022, the standard Part B premium amount is $170.10 ($164.90 in 2023) (or higher depending on your income).
If you need a service that the plan says isn't medically necessary, you may have to pay all the costs of the service. But, you have the right to appeal the decision.
If you have a Medicare Advantage Plan, you have the right to an organization determination, either orally or written, to see if a service, drug, or supply is covered. Contact your plan to get one and follow the instructions to file a timely appeal. You also may get plan directed care. This is when a plan provider refers you for a service or to a provider outside the network without getting an organization determination in advance.
You don’t have to pay more than the plan’s usual cost-sharing for a service or supply if a network provider didn’t get an organization determination and either of these is true:
- The provider gave you or referred you for services or supplies that you reasonably thought would be covered.
- The provider referred you to an out-of-network provider for plan-covered services.
